Update often breaks kernel

It often happens that on update, when new kernel needs to be built, I am getting error similar to 'cannot read' one below:

Then after reboot, that kernel can't boot and I must use fallback. However this only happens on pacman -Syu, if I build kernel manually with

sudo mkinitcpio -P
then it never fails.

It doesn't make sense to behave differently but maybe you can spot something. This is otherwise healthy laptop with zero problems.

  You had a microcode upgrade along with a kernel upgrade. By default  after kernel upgrade it does mkinitcpio only for the kernel but when microcode is also upgraded it has to do a global mkinitcpio not only for the kernel.

I'm having microcode placed as ignored so it doesn't get upgraded behind my back and do that nasty surprise. Every time you make a microcode upgrade you have to mkinitcpio -P, it's not only you.

Thanks for the clarification. It can be frightening for less experienced users, so if there is better way to solve it than blacklisting microcode it would be good thing to have distro wide.

 Making it ignored doesn't necessarily mean it's black listed it's just a safety net. Microcode gets upgraded 1-2 times in a year if at all so marking it ignored shouldn't impact much your work flow. Also when microcode receives an update pacman gonna warn you it's currently ignored so you just go to etc/pacman.conf remove from ignore then upgrade but remember to mkinicpio -P afterwards.

 Or simply be very cautious when you look upon pkg to be upgraded each time you do a pacman -Syu.

Sorry but I think you are being led astray. There is no "mkinitcpio only for the kernel" & "global mkinitcpio not only for the kernel".

What there is are presets under /etc/mkinitcpio.d

As standard there will just be /etc/mkinitcpio.d/linux.preset though there may be others if you've added them your self or *.preset.pacsave if you've ever edited /etc/mkinitcpio.d/linux.preset.

mkinitcpio -P will process all the presets in /etc/mkinitcpio.d (only with the extension .preset, not the pacsaves)

If you look in  /etc/mkinitcpio.d/linux.preset

The options for initramfs-linux-fallback.img specifically remove the autodetect hook

Why this should work when you run it manually but not when pacman runs it I'm not sure?
I'd start by trying to figure out which device that is.

You probably have lspci but if not it's in pciutils package.

I'd check your laptop is on the most recent UEFI / BIOS and also search the internet for your exact model of laptop and errors with the same device.

 If you mark microcode ignored in pacman.conf and the problem still happens yeah might be some other issue. See next time when you upgrade the kernel if it does it again could be something more than mc but I doubt 97% it's that case.

If I am not mistaken, 05:00.3 that causes problems is internal USB controller? However it only causes problems with pacman -Syu kernel upgrade, I have never failed to build kernel manually. Everything functions flawlessly otherwise and I was using this same laptop on Arch for a couple of years without any kernel build problems. I could bet it is software instead of hardware problem. Any ideas?

 I see some 'PCI Express...Renoir/Cezanne' device. PCI Express is known it gives errors at boot it's kinda oldish technology better unplug that device before boot.
